Insurance
Your insurance may cover some services. I am in-network for Medicare Part B, Freedom HealthCare, and Optimum Health. If you have Medicare, Part B, you might have some coverage for diabetes and chronic kidney disease. Most other services will be self-pay. If you have other insurance plans and conditions, we can provide a superbill (a detailed receipt) for you to submit to your insurance provider.
